The ICMJE recommends that authorship be based on the following 4 criteria:

- Substantial contributions to the conception or design of the work; or the acquisition, analysis, or interpretation of data for the work; AND
- Drafting the work or revising it critically for important intellectual content; AND
- Final approval of the version to be published; AND
- Agreement to be accountable for all aspects of the work in ensuring that questions related to the accuracy or integrity of any part of the work are appropriately investigated and resolved.

- The order of authorship should be clarified at the start of the project.
- All new projects should be discussed at the IR research meeting. If at that point another physician provides information that he/she has already collected data for the project, or otherwise feels that their contributions should be recognized in the order of authorship, it should be discussed and resolved. There will be no changes to authorship beyond this meeting unless there is substantial change in time and effort committed to the project.
- There are no restrictions on any physician conceptualizing or conducting a study. Given the infrequent occurrence of IR research meetings, projects can be initiated at any time and discussed at the next meeting.
- Primacy or number of procedures performed are not criteria for authorship, but consideration should be given to designating the practitioner who has performed the most procedures or initially introduced the technique to BCH as senior author.
- The criteria for first authorship should include:
- Writing the entire manuscript or conceptualizing the paper and assigning co-authors responsibility for writing specified sections of the manuscript.
- Performing the data analysis firsthand.
- If he/she first is a staff physician, submitting and handling all communication with journals.
- Physicians undertaking projects with the IR fellows assume responsibility for supervision, study design, review of results and editing of manuscripts. Fellows have been instructed to complete the first draft of a manuscript by the end of February. It is incumbent on the supervising physician to review, edit and submit in a timely manner. The aim is to have an accepted manuscript by the end of the fellowship.
- For diagnostic imaging or inter-disciplinary studies, only the physicians that have reviewed the images or patients should be included.
- For IR studies, all of the physicians that perform the procedure in question should be included. All authors should meet ICMJE criteria.
- If the primary author is unable or unwilling to complete a project, it may be reassigned to another physician following discussion in the IR research meeting.
- Disputes on authorship should be settled at the research meeting. In the event that the dispute cannot be resolved, the matter should be settled by a vote.
