Following placement or exchange of any type of venous access, the “CVC Pocedure Form” should be completed. This replaces the 
"Brief Op Note" that is required for other procedures.

• Confirm that the PowerChart encounter is correct for the date and location.
  • Under “Ad Hoc”, click “MD Folder”

• Select "CVC Procedure Form" and click "Chart"

• Complete the relevant information. Boxes marked in yellow are mandatory.

• Select the "Verification of Tip" tab. Select "Yes, under fluoro" and enter the location of the tip. Finally, click on the check mark in the top left hand corner to complete.
